Eligibility
You are eligible to enroll in Horizon PACE if you are:
- At least 55 years of age;
- Living in the designated 11 county service area, including Madison, Estill, Jackson, Rockcastle, Pulaski, Laurel, McCreary, Wayne, Clinton, Warren, and Barren;
- Eligible for nursing home level of care as certified by the commonwealth of Kentucky; and
- Able to live safely in the community with PACE services at the time of enrollment.
Enrollment
The PACE program becomes the sole source of services for Medicare and Medicaid eligible enrollees. Participants who choose to enroll in Horizon PACE agree to receive all services through the PACE organization.
Horizon PACE will be your sole service provider and will ensure access to any needed services by providers within our network.
All participants maintain the right to disenroll at any time.
