Patients can remain at home, in control of their lives and more connected to the community.

Personalized/coordinated care

Teams of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, aides and others – all of whom typically are employed by Horizon PACE – meet regularly to exchange information and solve problems as the conditions and needs of PACE participants change.

Upon enrollment in Horizon PACE, participants and their caregivers meet with that team. Their needs are assessed, and an individualized care plan is developed to respond to all of the participant’s needs – 24 hours a day, seven days a week, 365 days a year.

Our ability to coordinate the care of each participant enrolled in PACE is key to the model. PACE programs coordinate and provide all needed preventive, primary, acute, nursing home, and long-term care services so that their participants can continue living in the community.
